I currently use cable internet which requires an ethernet card which connects to the cable modem.

Will I need to buy an ethernet card for my DFI Ultra-D motherboard? I noticed that there are 2 RJ45 Lan ports - are these the same as the ethernet card? If so, which one do I use (does it matter)? One is "controlled by the Vitesse VSC8201 Gigabit Phy chip" and the other is "controlled by the Marvell 88E8001 Gigabit PCI chip"

Doesn't really matter which one you use as your cable modem is probably only 10/100 speed. However, in general, the one not connected through PCI is the one to use as it can handle a larger bandwidth (the PCI one is limited to the speed of a PCI bus, which I forget).
