Device to track alzheimer patient without GPS

A friend of mine lives in a country without GPS, he needs to be able to track an Alzheimer patient. He stated that only 500 meter is acceptable range.

I am thinking something radio but no clue if such a thing exists.

huh? gps works anywhere in the world. cheap android phone + google latitude + data plan.

anything that provides constant tracking is sure to have a monthly fee.

FYI, Skype is illegal in some countries.
