Decent free form making software?

As an assignment in out Systems Analasys second level class we're learning about input forms and output forms for our information systems ect.

We're required to use form making software. The school offers no software in that regard, he said go find a free trial of something. But I'm having a hard time finding a free trial of anything that will actually let me save the created form. Does anyone know of some good free form software that isn't full of spyware or something?
