Dead microsoft mouse, contact info for RMA?

I remember a while back there was a thread on here that contained info for easy rmaing of microsoft mice. Basically you scanned the bottom of the mouse and faxed it too them and they sent you a new one?

I searched and couldn't find the info, could anyone point me to the best place to RMA this mouse.
