Anyone know where I can get the $10 rebate form for the 50 spindle of 16x fujifilm from Compusa? I bought a spindle online, but I can't find the rebate form anywhere. If someone could send a scanned version or point me in the right direction I would really appreciate it.

I think this was a CompUSA only rebate and it is not online. This might help, found it on the CompUSA website:

Q: What if my local store is out of rebate forms?
