I can connect fine to xbox live wirelessly but when I try connect to other players while playing Madden it brings me to the screen but then says "can not connect to other player" (something like that). Do I have to change MTU settings? It passed the test. But on the xbox360 test connection NAT category it says "moderate".

Or does EA just suck in general.

I don't do much on xbox live so I haven't experienced any similar issues. however I did play a lot of Battlefield 2 & 2142 and with dd-wrt i did notice I would get disconnects and other network issues a lot more. I never did completely figure it out so I have been considering going back to a stock firmware from linksys.

You need to forward ports to your 360:
3074 TCP & UDP
88 UDP

You can manually open the ports and forward them to your 360's IP address (you should probably configure the 360 to use a static address if you do this). If DD-WRT supports port forwarding by hostname you can also give your 360 a name in the network settings and set the router to forward to that so it doesn't matter what local address the 360 picks up. Lastly you could try to enable uPnP on the router, but that's a crapshoot in my experience.

^ what he said....just open up all the ports and set the 360 to have a static IP with those ports opened. I have dd-wrt and it works fine

That's what I did. I was having random problems (not very often, though) with hearing other people in voice chat or connecting to games. Setting my 360 on the DMZ fixed all problems.

That's strange... Linksys WRT54G + DDWRT + XBox360 here and no problems. I can host Gears and Vegas and use voice chat without issue. My 360 is not DMZ'd or anything.

I'm also using a WRT54G but with the standard linksys firmware. I defiitely had issues in R6:Vegas and halo 3 until I set up the DMZ
