I've got a 9 month old Peek-A-Pom (1/2 Pekignese + 1/2 Pomeranian).

I take her out 3 - 4 times a day at regular intervals (Morning, Noon, Night, Late Night), and she still poops and pees in the house at least 5 times a week.

Today I found that she has been using the long-pile wool shag rug to hide her mistakes. Today I pulled up the rug to clean it, and I found pee spots that have ruined the hardwood underneath the rug.

What am I doing wrong? How long should it take to get her completely house trained?
 
Crate her when you can't watch her every second, for a couple of weeks and get her checked for a UTI. I'm asuming this has been going on since she was a pup?

Well, she was even worse as a puppy. Slowly but sureley she is getting better about holding it and letting us know when she wants to go out.

Still, there are plenty of accidents.
