Damaged Memory Stick Pro DUO 4GB?

I don't know what's wrong with thise Mem Stick,Is the Memory Stick damaged? I can write and delete files from it on PC and PSP, formating i can just on PSP but not on PC and i non't know why? I checked it for errors too.
Any help?
Do i need to return MEM stick?

I just want to make sure I am understanding this question. Everything works fine with it, you just can not format it from your pc?

Yes, i can't and format window doesn't shows File system and Allocation unit size parameters.
