Custom server types and IE launch scripts?

This is really two questions in one, but they are somewhat related.
First, I use Juno (I accept your sympathy). We actually went for the paid service for $5/month, and it actually works well. My beef with it, though, is that they insist on making you run their bloated program (30 Megs resident memory for a mail reader and IE client window? Sheesh) in the background to connect. This also includes when you run the connection from Dial-Up Networking, as Juno pops up as you?re connecting. After looking at this interesting little beast, I realized that they had created their own PPP server type. I figured that this has to be related to forcing the Juno exe to run, but what I was really curious about was how someone would create such a thing. Is it a registry hack, a driver, or what?
Second, I have a download-helper program, called DownloadMage, which can be set to open with Internet Explorer. It doesn?t change the shortcut or have a helper program that starts up with Windows, so how did they accomplish this?
I?m surprised my curiosity hasn?t killed me yet!
