Custom forms for Open Source CMS

I am looking for a way to allow data entry of community resources (agencies, providers, etc) and post it to a web site.
I would like to use one of the Open Source CMS's that exist. (IE PostNuke, PHPNuke, etc)

Any thoughts would be helpful.
