creating a signup informaton sheet...

II am looking for small slips...perhaps four or six to a page that include the following:

Name:______________________
Address:________________________
City:____________________________(etc.)
State:
Zip:
E-mail:

any ideas on how to create something like this in some sort of standardized way?

OOO! This is for office work and not for a website? Adobe page maker should work fine.

Why the hell would he put 4 to 6 of the same form on a webpage? He wants to print them out and cut them up.


I'm pretty sure Word can do it. Use a business card template or something.

Exactly.

MS Word works jsut fine. Just setup a blank label template and type in whatever you want. Then have it start printing.
