Crazy Artifacts. PSU or GPU Problem?

I've had my 460GTX for about two years now and I've just recently been getting intermittent artifacts. What I mean is, I could be playing a game, and suddenly artifacts pop out everywhere. I get a "display driver has failed" and the game crashes back to desktop. However, if I open the case up and replug the power connectors to my graphics card and reset my computer, it often fixes the problem... for about two or three days and the artifacts reappear.

Is this most likely a faulty PSU or a faulty GPU?

I ran OCCT GPU 3d I get thousands of errors instantly, but I do not know how to comprehend the OCCT result graphs. Could someone explain which part is causing the issue?

Off the top of my head:

75% chance of defective GPU
15% of defective RAM
10% of defective mobo
5% of defective PSU

Is the GPU overheating? Perhaps the ventilation holes are clogged by dust or cat.

If you have a replacement video card somewhere, I would check with that.
