Crafting payment agreements?

Need to find some simple fill-in-the-blank payment agreements, for monthly/weekly payments. I could write my own, I'm fairly good in that dept, but if there are any premade ones it would be nice to find them.

I have two clients currently making payments, and one client that, unfortunately, hasn't made a payment in a year.

Ohh, I LOVE those !!! I had to threaten to pull the hardware I put into their PC last year from such customers.

Would have made for a helluva funny sight .. me banging on the door, yelling "I KNOW YOU ARE IN THERE !!!!"
