counterstrike mouse lock-ups

This used to happen on certain servers, now happens on most...

When a lot of people are nearby the mouse gets completely unresponsive. Still can move sideways, but can't turn / aim. The lock-up lasts about 1-2 s, just enough to get killed...

all net_graph numbers look OK, no ms spikes, no choke, no loss

Any ideas?
