For an ATA100/133 40-pin/80-wire IDE cable, master drive goes in the other end (black connector) and slave drive in the middle (gray connector) right? Or is it the other way around?

Also, how would you setup a CDRW and CDROM on a single IDE channel?

it doesn't matter as long as you use the master/slave jumpers on the drive.... traditionally in a CS(cable select) environment the master would be the end and the slave would be the middle.... that applys for all IDE configs i believe.
 
Yes. Blue to mobo, black (end) connector is master, grey (middle) connector is slave.

As for those being on the same channel, it probably doesn't make a helluva lot of difference which is slave/master.
