Cooling, stability & designing heatsinks..?

There are some issues that's been bugging me,like if proper cooling is so essencial to a stable system, why don't manufacturers of coolers/heatsinks make'em so they fit the hole surface on cpu ?
-I mean, the only surface on the cpu that's in contact with the sink, is the core in the middle, right?
Wouldn't it be better if the entire chip had been cooled down?
Please can someone explain? :confused:

The rest of the chip only contains circuit traces embedded in epoxy and/or varying types of substrate. That part doesnt get very hot...
