Answering my own question... just like a page counter, refreshing ups the count.

Any hit on the page will increment the count.

And yes, it's because it was just turned on.

People's view counts aren't a good indicator of anything. People that spend a lot of time here will have a HUGE number of views due to page refreshing, but their post count may not be that high. Other people, though I won't mention any names, post in every thread they view.
