Converting a Word document to field names in a database?

I'm trying to convert this checklist (mainly yes/no and some #s) into a database so that the info can be entered and used to generate various reports, spot trends, etc.

Only problem is... I have no idea how to do this.:eek:

Can anyone give me some advice on an efficient way to accomplish this?

Also, if there are any programs that can do this automatically, please let me know.

Not sure, but I think you can goto: view>toolbars>database and then clk on the form button. Suppose your data needs to be in columns with tabs or some character between.

I also think you should be able to export a Word file to Excel, but I can't seem to find how with my version (Excel97 sr2). I have done this before, but long ago with an older version of Excel.

Try this: highlight your data, press ctrl-C, goto Excel, press Ctrl-V. Data should transfer. You may have to move data around to suit.

Or goto 'help' in Excel. Type: 'import data' and see what you get.
