Confused about claiming my upgrade to Vista

I recently bought parts for a new build, along with a Copy of Win XP OEM. The copy of XP came with an upgrade redemption form for Vista. I want to get the upgrade, but I have a couple of questions about the correct way to do. The instructions I received seem inconsistent with the upgrade form I have. I don't want the risk of having my upgrade denied because I did not claim it correctly!

I already went to the website, entered all relevant information, and have my confirmation number. The instructions on the form tell me to send the redemption form and order confirmation number to the address shown on the form itself. My form, however, has no address on it (it says "mail.... in the same envelope to:" and then there is a blank space where, I guess, someone should have entered an address but didn't). Where should I send my form? Is there a fax or email I could use to send everything? I am in the US. The redemption form also has a serial number in a box at the top right corner, which I provided online. I am not sure if this makes any difference.

Separately, the upgrade form tells me to enclose the receipt from my "PC preloaded with Windows XP" from my "authorized system builder". Since I built this myself and got most of the parts (Mobo, Ram, CPU, Monitor, HDD and Win XP OEM) from Newegg, I am assuming I just need to enclose the Newegg receipt for all the parts I bought, even though I am not an "authorized system builder" and I did not buy a "PC preloaded with Windows XP".

Can anyone confirm that all this is correct? Can they deny the upgrade because I am not an "authorized system builder" and I did not but a "PC preloaded with Windows XP"?

i thought that the whole point of the OEM version of Vista is that it makes *you* into the system builder (authorized)
 
No. You will not be denied of your upgrade. You are the system builder. You just need to send your proof of purchase from Newegg. I faxed my Newegg invoice to them using the fax number listed on the form. You can also email your inoice to them. No need to send the form to them since you have already completed the process online.

I did both, sent the invoice through fax and email. It took a very long time for them to process the order. I received the DVD about two week ago. It took over two months for them to process my order and mail the upgrade DVD.
