Long handle screw driver and glue on the tip. Attach to VGA knob. Wait for glue to set. Turn. Break glue off screwdriver.

These things can be a bitch to turn especially if they are tightened. I can't imagine you're going to get enough torque trying to be grabby with the outside, AND still have room to turn. You'd also only get a 1/4 turn before resetting with anything pliers-like.

edit: oh tighten... Hmm, you only need a couple turns so it doesn't fall out. I'd try the wire nut suggestion.

Heh, I had an uncle that does stuff like this. Weld nuts onto stripped screws and stuff like that. I could see him stripping the plastic on the fasteners, cutting the heads off of some screws and welding or soldering the head onto the cable fastener.

Hot glue in a nut driver would probably do it a little less permanently.

Might as make make it permanent in case he wants to take it off. If he has a dremel he could make a slot for a standard screwdriver pretty easy. I was just saying I could see my uncle welding something for this, because, its just something he would do. Look around for some scrape the just make it work. Heck, I could see him attaching a hook to the tv and securing the cable with a rubber band.

Really he needs to replace the screws in the cable, or replace the cable. Install is one thing, but what about removal?
