Con Not Format Floppy

im trying to flash a bios on another computer and i need to format a floppy, but when i go to format it it gives me the error cannot format, and it cant read the floppy either, but according to the BIOS install program it extracted it fine onto the disk. I need to get this done so the mobo in this other computer will work right.

There are several threads here on floppy problems - use the advanced search and include the Gen Hdw, Peripherals, Mobo and Tech Supp sections. Just about every possible cause was discussed.
