Computer hangs in 3D games

I've been having a very frustrating problem with my computer thats dragged on for many months. I've finally decided to solve this problem once and for all, no matter what it takes. I'm hoping someone here can give me some suggestions. At this point I have tried literally EVERYTHING short of buying new components.

The problem is only when playing 3d games, any 3d games. Within 15-45 minutes, my system will freeze, blue screen, lose signal to monitor, reboot itself, hang in a sound loop, etc.. This ONLY happens when running FSB at 200 mhz. Here is my system spec's:

ABIT NF7-s v2.0
AMD 2100+ tbred B, Clocked right now at 2.17, it runs reliably up to 2.3 w/extra vcore
ATI 9800 non-pro, runs reliably at pro speeds
2x512 PC3500 Mushkin Level 2
1 WD 80GB SE
1 WD 74GB Raptor
Liteon Combo DVD/CDROM/CDRW drive
3.5 inch floppy
Antec Sonata w/ 380 watt TRUE380S

Now, if I clock the system to 166 FSB all is well, even after overclocking the CPU by multi, and 9800 non-pro to pro speeds. Memtest at 200 FSB reports NO ERRORS, have run it many times overnight. Prime95 runs perfectly fine as long as the vcore is adjusted based on the CPU speed setting, even at 200 FSB. The problem ONLY happens in 3D games.

Now, I have taken note of my PSU +5 and +12 rails. The +5 hangs around 4.9, and the 12 at around 11.8-11.85 during normal operation. I've been thinking could this be the problem? I kinda just forgot about a PSU problem when I bought the Sonata, are those voltages a little on the low side? Maybe these are dropping even lower while running a 3D game (i.e. stressing system)?

I'm not totally convinced this is a PSU problem yet, but can someone recommend a good PSU that is also QUIET?

[EDIT] P.S. here are some of my voltages/temps:

vcore = 1.70
vddr = 2.9
+3.3 = 3.25
+5 = 4.92
+12 = 11.86

CPU temp = 39.5 C
SYS temp = 27 C

hum, if it only happens in 3d games maybe the problem lies in the Vid card cooling. If I remember correctly changing some of the FSB will also change some speeds in the AGP ports and video cards. (I know that sounds very incoherent, but im a little sleepy now)
