computer freezes randomly, screen goes to a single frame with vertical lines

I have an asrock visionx 240d which I installed windows 10 on. It's connected to my tv via hdmi. Every once in a while it just freezes with a single frame, usually black background with a bunch of vertical colored lines. The lines are the same color as each other but may be different color on different freezes. It seems to happen more often when doing something slightly more performance intensive (eg playing civ5), but can happen randomly when just browsing the web too. It's a fairly recent windows 10 install and I haven't downloaded or installed much of anything.

What should I do?

edit: installed some programs to monitor cpu and gpu temps, which mostly seem fine.
