I have a CodeGen case, dunno if they still even make it, that meets those requirements. It has a blowhole fan in the top, and I added an 80mm fan to the front, it already had mounts for it. I had to drill additional mounting holes for the HDs to move them back to accomidate the fan, and I drilled ventilation holes into the bottom front plastic of the case.
It's a mid-tower with 4 external 5.25in bays, 4 internal 3.5in bays, and two 3.5in external bays.
Oh, I just noticed you mentioned 120mm fans. These fans were all 80mm.
