Looks like a photochop.
But if its real I'd say it just means you used a bunch of combos in the round.

But I've had many combos in a round and never got "combo whore".
And I think you would need more than 56 kills. Unless you had a custom map with lots of pills.

The Combo Whore reward is real. You have to kill a bunch of people with shock combos. It's just like Flak Monkey.
