clean install FAQ

I was checking through the faq's and realized (I may have missed it) but there isn't one on how to format and do a clean install of an OS, in the past couple of days I have answered this question several times, and thought maybe we could add it to the FAQ's lmk if I am out of line is asking.

Do you have one written? If so pm andyhui and he should be able to read over it and put it up if it covers everything.
