Circuit City rebate question

I purchased a hard drive from Circuit City and looked at my rebate forms that printed out at the register. The asshat prefilled all of my personal information without verifying it with me. As a result, they have my old address prefilled on the rebate form. Will it be ok if I just print out the rebate form online and send that copy in instead of the rebate form printed out at the register? Or do I have to send the rebate form printed out at the register in as well?
I tried calling the 888 number but I can't seem to get ahold of any humans.

Also I can't remember if I've asked this before but do you guys normally staple the UPC to the rebate form?

2. I do - along with the receipt (whatever form it is in) and any other documentation required - all stapled together.

I prefer a paperclip (less chance for ripping when it's examined at the rebate center), but definitely make sure the smaller items in the envelope are attached to the large items somehow.
