Chimei CMV 221D Newegg Rebate Form

I'm not entirely certain where this post belongs, but this seemed like the best place.

I'm looking for the Chimei/Newegg rebate form for the 221D that ended on 8/13. I bought the panel about 10 days ago, and Newegg changed the form to a newer rebate. I've searched online, and I can't seem to find any copies of the older version of the rebate. To make things even better, there's a 15 day cutoff for the the rebate postmark, so I'm almost out of time.

If anyone still has the old PDF for the rebate, I'd be extremely grateful.
