Driver was probably changing to the fast lane but did not see the trailer. Shouldn't the trailer be in the right lane though?

impossible. The truck was slightly ahead, and the nissan was in the shadow of the truck. Blowout maybe?
 
How does a Nissan van drive a semi into and over the center divider?

Looks like the Nissan slammed right into the front wheel of the semi. Even with a small car that's a lot of force. There was maybe 2 seconds for the semi driver to react and the front suspension most likely got tweaked along with a car still driving into it. I don't think the car helped any pushing the truck over the divider other than point it towards it at 55+ mph lol.

I sure hope the driver in the Nissan got their text message sent!
