cheap MS optical mouse troubles

I got the following cheap MS mouse/keyboard combo for a machine I just built:

It's a combo I've got several times before, but I guess I've never actually put it on an NF3 250Gb board before.

The mouse works fine almost all the time, but twice (in about 3 weeks) it's stopped responding until I unplug it and plug it back in, and sometimes it just flips out and goes to the top-left corner of the screen. I've tried plugging it into two different USB ports and even using the PS2 adapter, but it acts the same.

Is there anything I'm not thinking of that might be doing this, or should I just RMA the mouse?

The system also has an HP all-in-one printer and a PS/2 keyboard hooked up to it.
 
We use those at work exclusively, we go through about 5 a month. They are dirt cheap. We pay 11 bucks for em so its no big deal. I have managed to figure out why 90% go bad though. It appears the USB metal casing get bent a bit and I can squeeze em tighter. The other 10% that go bad are because of wire crimping. Those I just toss.
