CF Card Won't Format Properly

So I popped my gf's CF card into our multi card reader yesterday and for the first time put it on a new laptop. The laptop wouldn't recognize the reader properly, so we hooked it up to her desktop and it said the card was empty; her camera said the card wasn't formatted properly.

Using the camera's format function doesn't work. It tries to take a pic but says it can't write to it.

I format it in Windows thru the card reader (16MB card, so it only formats FAT and not FAT32), camera says its not formatted.

Suggestions?
