Cellular Insurance Question

Hopefully some of your have experience with cell phone insurance and can answer my question:

If my cell phone is broken, can I just call my provider (Cingular) for a replacement?
Or do I need to goto a Cingular store first for written proof?
 
i think my boyfriend just called sprint when he lost his. i imagine they have computers with the insurance info on file... that's just my guess.
 
when i claimed insurance with att, i didnt need proof. it was through lotline (or lockline?)... they just sent me another phone

okay--thats the insurance I have under cingular also. good--too much of a hassle to find a cingular store nearby.
