I let it's Cleaner tool remove everything it found (cookies, Flash settings, and logs), but am nervous about letting it 'fix' everything it's Issues tool found. Can it cause problems or make a change I wouldn't want?
Things it found are missing shared DLL's, unused file extenstions, open with application issues, invalid default icons, activeX/come issues, installer & uninstaller reference issues, application path issues, old start menu keys, and missing shortcut references.

Anyone care to take a couple minutes to tell me what exactly it will do to fix each of the issues?

If these were actually all problems, wouldn't I have noticed before?
Also, if I've made my own changes to say start menu, or open with, etc., is it going to change it back to the default?

If you aren't encountering any problems right now with programs unable to find a path or .dll, I'd leave everything as is. If you want to clear history, cache, etc.. just do it with Windows. In the past I would repair all those things that it said were wrong but in the end I was no better or worst off.
