CINCINNATI -- Carson Palmer's knee injury was "devastating and potentially career-ending," involving numerous ligament tears, a shredded ligament, damaged cartilage and a dislocated kneecap, his surgeon said Thursday.

The Cincinnati Bengals quarterback tore ligaments in his left knee when he was hit by Pittsburgh's Kimo von Oelhoffen on his first pass during the Steelers' 31-17 playoff victory Sunday.

The team announced that he had torn the anterior cruciate and medial collateral ligaments. The damage was much more extensive and severe, but Dr. Lonnie Paulos said surgery went well and Palmer could be back for the start of the season.

Palmer had surgery Tuesday in Houston. Doctors used grafts from other parts of his body and donated tissue to fix the damage during an operation that lasted more than two hours. Palmer headed back to California on Thursday to do his rehabilitation.

"It's not just like it was a torn ACL," Paulos said Thursday, in a phone interview from Houston. "It's a magnitude more difficult to recover from and repair. It can and has ended careers, without a doubt.

"However, I feel very comfortable with Carson as an athlete and the heart that he's got. In the end, that's the bottom line. I can see the look in his eye already. He's ready to get going," he said.

Paulos, an orthopedic surgeon who has worked with the U.S. Ski Team since 1983, replaced the anterior cruciate ligament, which runs through the middle of the knee and provides stability. He said the medial collateral ligament, which runs along the side of the knee, was damaged "real bad."

"On a scale of one to three, it was a four," he said. "It was off the chart. It was pretty badly damaged -- shredded is the better term."

The kneecap dislocated when Palmer was hit, damaging tissue around it. There was also some cartilage damage, he said.

Paulos was able to repair the knee without removing pieces of cartilage or soft tissue, a good sign.

"The things that were torn could be repaired," he said. "They were not torn beyond repair. So he's got all his parts in there, which is good. We're optimistic, actually."

If rehabilitation goes well, Palmer could be running in a couple months and might be able to play in the first regular-season game, Paulos said. The 2006 schedule hasn't been set.

Palmer has worn a protective brace on the left knee since he sprained it near the end of the 2004 season. The knee bowed inward on von Oelhoffen's hit even though Palmer was wearing the brace.

"The brace didn't function well in this environment and should have done better than it did, frankly," Paulos said.

The plan is for Palmer to wear more substantial braces on both knees when he returns.

"No brace is perfect," Paulos said. "No brace can prevent every injury, but they do help."

Paulos saw the replay of the injury and wasn't surprised at the extent of the damage it caused. Palmer has absolved von Oelhoffen, saying he didn't think the lineman was trying to hurt him. The lineman said he was trying to sack Palmer, not injure him. He wasn't penalized for the hit.

Palmer made the Pro Bowl in only his second season as a starter, throwing an NFL-leading 32 touchdown passes. The club extended his contract through the 2014 season.

Jon Kitna, who has been Palmer's backup and mentor the last two years, can become a free agent. Palmer's injury will force the Bengals to make sure they have another reliable quarterback on board.

That sucks for the Bengals after signing him for like 10 years. Im sure Palmer couldn't care less about the money though. He wants to play ball.

For clarification, "The Experiment" was a good thing. I had him in one of my leagues since the rookie season, another owner had him in my main league. We were very optimistic about his shining future, particularly watching him in action this season.

He needs to retire, now. I know he still wants to play, but that knee will never be that strong again. Another injury may well significantly impair his ability to walk normally.

He's a young guy that has plenty of money and needs to go into broadcasting....and save that knee for playing in the yard with his kids.

Speaking from experience, that injury REALLY sucks. Now Im by no means on the level of Palmer in terms of athletic ability, but I would like to think I am/was a pretty good athlete. Its been about 7 months from my surgery and my knee is no where near 100%. And I have rehabbed very hard/often. I hope he's able to recover, but make no mistake... its not easy.

He just signed through 2014 IIRC...he if can play, he'll be on the field.

uhhh, this is the NFL guys, not the NBA. if it were the NBA, then ya, he's set for life, but this is the NFL where, if you don't play you don't get paid.

a much smaller percentage of his total contract is guaranteed, unlike in the NBA where the entire thing would have been guaranteed.

he will be fine. theyre making this out to be more than it really is. yeah, it was a serious injury, but they were able to repair everything. besides, its not like hes a scrambling qb. as long as he works hard at rehab, carson should be back shortly after the season begins.

no, what would suck for the bengals is IF the entire contract HAD been guaranteed and Palmer WAS out for life. that would truly suck (refer NJ Nets and Jason Williams).

no, what would suck for the bengals is IF the entire contract HAD been guaranteed and Palmer WAS out for life. that would truly suck (refer NJ Nets and Jason Williams).

It would still suck for the Bengals as they NEED Palmer. THis was evident in the playoff game.

its impossible to know for sure until the surgeon actually opens you up. MRI's give you a very good indication, but they are only that... indiciations.

That truly sucks.

I wish there was a brace that athletes in contact sports were "required" to wear that would help prevent this. It's so fvcking stupid, because these injuries happen regularly, if not on a daily basis. No one wants these guys to get crippled for a game. We all say it, but it's all in context of the game though: "Cream his fvcking ass!!!!"

Again, there should be some kind of protective brace that must be worn to protect the players knees, regardless if it inhibit's the player's speed or ability. It has to be done. They wear helmets for their heads, are their legs any less important?

This injury has been recurring ever since this sport (and other like-minded sports) has been invented, and it needs to be addressed. We can't keep allowing the finest & most fit atheletes to be crippled for our entertainment, that sh|t went out with the Romans, didn't it?

I don't think he's arguing that point, just the fact that they jumped the gun and said that he'd be back for training camp before they even sliced it open to take a look.

Willis McGahee knees was much, much worse...and he's playing RB carrying the ball 20+ times a game. AFAIR, McGahee's injury was: 3 three ligaments inlcuding ACL and MCL completely ruptured...significant torn cartilage and bone chips removed from the knee.

Carson will be fine... I think Paulos was being a bit of a drama queen and was trying to get himself some free advertising.

He's a QB with a great arm...he'll be back even if he can only move like Marino. If he was a WR or a RB he would be done, but thankfully he isn't.
