It's a f2p game but from what I have seen so far what you get by paying is not a big issue as all it does is either give you more equipment that you can get over time, some different avatars and unlock some extra campaigns. The campaigns can give decent rewards but you can get access to a few of them for free with the money they give you.
You unlock the multiplayer option when you get around level 6 and get some free equipment from it with a free starter deck. In multiplayer you gain the ability to pvp against other people for loot chests, play custom campaigns or even make your own but hopefully they add some kind of co-op so you can play with friends for rewards.
Otherwise it's just about equipping your characters with the right equipment as each type provides different cards to play during combat.
Just a word of warning but the game just went live a day or two ago and it got flooded by a ton of people so you may have to wait in a queue at times to get in until they fix it as penny arcade mentioned the game.
A couple screenshots taken from their website that people that have played D&D may recognize a bit and the bottom cards you see are what you use to play the game.
Make sure you have at least one dwarf in your party or you are not cool enough to play.
