Can't find Nikon's rebate form for Dec.2004 for the Coolpix 5400

Nikon's current online rebate form for the Coolpix 5400 is
for Jan.1 2005 ~ Mar. 31 2005.

I purchased mine from Amazon Dec. 27 for the valid Dec.
rebate, but can't seem to find the applicably dated rebate form.
Is it OK to use the 2005 form, or do I have to use the form
that specifies December's date?

Pls Help!!!!!!

Rebate for coolpix 5400, purchased Dec 1 to dec 31,2004.


Remember to get a move on, as the rebate has to be in before the end of this month.
