Assume it has a Supervisor IV card.

If so, what part do I need?

no.

You need a router with a csu.

unless cisco's come out with a WAN card for the 4500 (which I doubt)

Yes, you can. You need the WS-X4604-GWY linecard and your choice of WIC/VWIC. However, that setup comes at a STEEP cost. The list price of the linecard and a T-1 WIC is $8500. For that kind of money, you can get SEVERAL wan routers (2600, 1700). So from the price perspective, its not worth it...

You could probablly get two 2621s for about the same price which come with built in t1 csu ports and would offer the same level of reliability.
