Can someone help me out with a videogame "mission failed" image?

In my mind's eye I'm picturing a tan piece of paper like a file folder with your mission on it. If you fail, you see that screen again and it details how you failed your mission. I thought it was from Goldeneye, but google image search isn't turning up anything.

The screenshot doesn't even need to be like that, really. Any sort of mission/mission failed screen will do. If you have a game with a good screen like that can you take a screenshot for me? The higher-res the better. Thanks.
 
I could possibly photoshop whatever text I need into the picture, the content of the picture is more important. A picture of a dead person lying in a pool of blood (haven't seen the RE screen, just guessing) would be inappropriate for my purposes.
