Can anyone offer any more insight into "iDevice is disabled" (not too many login fails)

I sometimes get customers wanting help with their iDevices being disabled for no apparent reason and the only solution I've ever found to work is to hook it up to a computer and have iTunes wipe the device and start over. I assume it's Apple's way of not wanting to admit that their products inexplicably fail.

Can you offer any more insight in the error that you're receiving? Since i know it doesn't literally say "iDevice is disabled" on the screen, what is the issue? How confident are you that it's not a failed login issue?

The last time I encountered the problem (the other day), the customer used to have a passcode set up but it was (according to them) removed three weeks before the "iPhone is disabled" message appeared.
