Can anyone help with a photoshop?

So let me just start by saying this photo is quite personal to my girlfriend. It is one of the few images she has of her dad and today is the anniversary of his death.

This particular image is actually a film photo that has a white smudge on my girlfriends nose and my meager photoshop skills aren't up to snuff to digitally fix it. So I turn to any member who can help with this. I'd be ever grateful and my girlfriend would be truly truly grateful. This would quite literally mean the world to her.

What do you want besides the white smudge?
Scratch and dust? Color? Cooler, warmer? Sharper? Softer?
Be specific, I dont wanna start until I'm sure.

Actually I been trying for a couple hours and frankly I cant fix the white smudge. Everything I do makes the pic look horrid. Truthfully I'm not that skilled. I got used to an old version of Photo Paint and I can't even do the same things in PaintShop Pro x5. In fact even the dust remover is kicking my butt.

The quality is pretty poor and there's not a lot of pixels to work with, but hopefully this is a bit better. Tried to clean some of the gunk out of the sky as well.

My .02... I removed the post and the splotches... fixed the blob on the face and did some noise reduction.

Not gonna add color to a sky that probably didn't have color to begin with.

reconstructed the noose after spot removal, tilt/shift blur for added mood, color balanced original photograph, now image is more daylight balanced. removed the object on the left, replaced area with "content fill", added lens flare from top left corner according to original photograph's lighting, and some light dust removal.
