Input: 100-240 VAC .6A, 50/60HZ
Output: +5V 1.5A
+12V 1.8A

This is used with an external Firewire/USB2 enclosure I purchased at Compgeeks.
I've done a number of Google searches and I come up with a ton of references to the adapter but nowhere to buy one.

 
Find a replacement that has the same +5v/+12 dual output with same or better amperage rating.

# 680-WRI3021-D5 it's a 5v/2A 12v/1.5A It has a 5pin output din. If yours is just the normal type din they probably have an adapter with the output connector you need.
