Calling all teachers

Please forgive me if OT isn't the best place for a question like this but it's kind of an obscure request as it is so I wasn't sure.

I need to create a small quiz which can produce (print) a basic "certificate of completion" at its conclusion, could anyone offer any recommendations how I might go about doing this? Your help would be greatly appreciated!
