AFAIK, they are not placed under your cast.

They only eat dead flesh and they aren't maggots from house flies, they come from a type of fruit fly if I'm not mistaken. They can actually keep wounds very clean. Think of it as a nurse cleaning your wound every few minutes without any of the pain of being scrubbed.
