At the least, you go deaf. You'd definitely fall off your feet. (I've read that you can feel the ship moving sideways when those things fire.) You'd probably be blown overboard by the blast.

It depends on how far you're from the end of the muzzle obviously. Hearing protection is absolutely required. If you're too close, however, hearing damage is going to be the least of your concerns!
