Blank screen and Disk I/O error

Ok here it is, I have a customers computer I'm working on. When I cold boot the PC it says there is a Disk I/O error. Now, I have tried switching cables on the HD and Floppy, I changed the settings in the CMOS and BIOS, I even changed the HD. Also when the computer has to be restarted, the screen goes blank. I tried switching video cards and some cmos settings nothing works.

Well, looks like most of the system's been swapped out. Maybe the motherboard's died. What kind is it?

The whole system is old. it's a P-133 blah blah blah. The mother board, I have no idea. It was custom built way back when. It probably is dead. I just wanted to make sure I tried everything. I guess I will swap out the mobo just to make sure. Thanks for your time.
