The best cooler is the Thermalright HR-03, but it's big and costs more. The Accelero X2 is a good choice if you don't want to spend much.

This has been covered in detail a while back over at the Rage3D forums. The stock cooler on the X1800XT is actually not bad. The Accelero X2 does work better, but it also significantly raises the overall temps in the case since the hot air is no longer vented out like it used to be with the stock cooler. My own personal opinion, unless the stock cooler is failing or defective, I would just stick with it. It does a pretty good job all things considered.

The stock cooler is also pretty loud, even at idle.

For me, it's only loud at initial spin-up. I heard lots of stories about how loud the fan was before I bought my X1800XT, but was actually surprised to find it fairly quiet. The rest of the time, the fan never gets above 30-40% even in the most extreme, in-game cases. At idle, I can't hear it above my case fans.

It depends on how loud the rest of your system is. Mine was pretty quiet and maybe a foot away from my ear, so I noticed it.
