Basic Question about Haswell VCORE

I have my 4770k at 1.21x V in BIOS on an Asus Z87-Pro.

You can see a shot of HWinfo64 where you see the voltage I set manually in BIOS on top.

I am however interested in the "real", dynamic Vcore that is on the CPU.(I understand this is also affected by C-states). The voltages below. (In CPUID Hwinfo this is called VIN4 by the way)

So in the picture you see my idle vcores which are only a few mW. But under load, say OCCT testing, all the VCores go up to currently exactly 1.232 V

The issue is I have no idea what affects this actual Vcore voltage and why it is HIGHER than what I manually set in BIOS which is 1.21x.

I dont use adaptive or offset or anything. I also have Load Line Calibration at 8. I don't understand where the additional voltage comes from, how my 1.21x V translates to 1.232 V actual Vcore voltage.
