Bad power supply? (screenshots included)

First, make sure that you have the power line properly and securly connected to the motherboard. Second, get another PS, those Antec's aren't any better (IMO, worse) than generic. I had two fail on me, never had a ps crap out on me before or since.
 
62c cpu temp & 42C case temperature. :Q

You better get a few case fans in there before your wires start melting.
 
Keep in mind though, this is an Antec 400W AMD approved power supply, not your run of the mill generic crap

I have a run of the mill generic crap power supply....on the side it says "Generic Crap 350W" 😀..quite happy with it, voltages not excellent but good....plus it has an internal cooling fan to take heat away above the CPU 😉

But back to the case at hand....yes it does look like that power supply has gone bad and like Budman says, extra cooling is advised. You could also try downloading a 3rd party hardware/voltage monitor...is your board Via based?....try the VH Monitor.

I found out the solution to the problem. I notice that the voltages only went haywire with Motherboard Monitor 5 installed. After I uninstalled it the voltages went back to fine.

To double check, I reinstalled Motherboard Monitor 5 and rebooted. Instantly PC Probe started going crazy. MBM 5 reported the same voltages.
