You need an "n/a" choice for a lot of those, or can I just leave them blank?

I just left them blank.

Also OP, the question about pant size is kind of weird depending on who the survey is for. Pants here in the US (for guys) come in numerical sizes, not S-M-L-XL sizes.
 
Done.

Most of my answers were "other" or just left blank. It seems like that survey is geared towards little people who care about fashion and live a block from their workplace.

[edit] Your friend writes crappy surveys. What's the difference I'm willing to pay for eco-friendly apparel? Why isn't $0-10 an answer?
