ATI Radeon drivers crash on normal clocks?

My specs are below, built in June 2012, still running Catalyst 12.8 drivers.

I OC'ed it in November from 870/1200 to 1200/1500, almost a 50% increase, and I tested it with furmark/heaven and it was stable. I got a few crashing drivers while OCing, but after hitting 1200/1500, I was satisfied and bumped it back down to stock clocks.

I recently bumped it back to 1050/1300 so I could play Skyrim with 30+ mods installed and with that small overclock, my temps never hit anything above 72 Celsius.

The drivers crashed yesterday while playing planetside 2, and is 1 crash an indicator that my video card is failing?

In addition, there are wierd things happening on my computer, the first one probably has nothing to do with the video card but I'll post it just in case, chrome freezes the entire computer for maybe 2 seconds, and then proceeds as normal.
And the other wierd thing is I've had the entire game go white in 2 games so far. The first was sleeping dogs back in December, the whole screen just went white, and the second was today in Planetside 2, any textures that were rendered beneath my feet started flickering, while sky textures stayed the same.

1.Maybe a flash thing not certain.Update your flash player or you can also disable hardware acceleration to see if that helps.

2.Video drivers crash doesn't necessarily mean your gpu is dying.First ensure that you get no graphical glitches while playing at stock settings.Then oc the core and test for stability and repeat the same for memory.If your memory clock isn't stable the error correction routine in Gddr5 will kick in.

Did you try the newest driver? Or still running 12.8?

Also, sometimes some games will not do so well with an overclock but others will. For example, my OC was Unigine Heaven stable, but Black Mesa Source of all games (Half Life 1 lol) would crash every once in a while until I upped my voltage, so don't assume that being Heaven stable means it is stable in everything.

I updated to the newest non-beta drivers and now they crash a couple times a day. Somewhat frustrating after going 2 years with Nvidia and not having that problem.

The freezing in Chrome could be a virus/malware scan that checks if links are "safe". I use Avast and it does that quite regularly.
